At High 5 Ballooning we strive to offer our passengers the highest quality of service available. This includes informing our passengers of all company policies and procedures.

Below are four letters for you to read

Our insurance company requires that every passenger making a reservation is given the opportunity to read and understand the release document on the day that the reservation is made.

You are not printing out or signing this document. Your acknowledgement is all that is required at this time. We will supply every passenger with an original copy for you to sign on the day of your flight.

Passengers that fail to read, fill in and submit this form will not have a completed reservation. The date remains open and can be sold.

Once we receive this form, we will send you a confirmation number along with the meeting time and a very important reminder to call us the night before your flight at 8 pm for a check on the weather and final flight confirmation or cancellation.

Please remember to make this very important call. We do not make preflight confirmation calls.

Please fill out the required passenger information form. Completing all fields is required. We will compare your entries with the information you supplied to us when you called to make your flight reservation.

Check the I Agree, Submit Form box to indicate that you and have read, understand, and agree to all of the information contained within the four hyperlinked documents on this page as well as the cancelation policy. Click the Submit Form button to send it to us. When we receive it, we will send you an email with your confirmation number. Once you have that, your reservation is secure and complete.

I have read, accept and agree with the terms of, the Release of Liability and Assumption of Risk Agreement, What You Should Know Letter and Cancellation Policies. I have also received Directions to the meeting location.

Cancellation Policy - Customers will be charged the full fare ($200 per person plus any additional weight or private flight surcharges) or have any pre-paid flight ticket voided if they fail to arrive for their flight, late arrivals, (more than 15 minutes from the agreed meeting time) or cancellations made less than 7 full days prior to a scheduled flight.

Pre-purchased flight tickets are non-refundable and must be presented for redemption before every flight. For flight tickets that were issued without payment. We will collect your fare on the day of your flight.

We reserve the right to cancel any passenger or flight where safety may be a factor.
